Forms

  • Express Registration Instructions Sign (pdf) -- This sign is used at events with express registration.
  • Membership Form -- For the form to apply for or renew your TOC membership, see our Membership page.
  • Registration Log (pdf) -- This form is used by the Registrar to log participants during events.
  • Timer's Log (pdf) -- This form is used by the Timer to record start and finish times during events.
  • Event Fees Poster (pdf) --  This display poster displays the standard fees for your event.
  • Today's Schedule Poster (pdf) -- This fill-in-the-blank poster displays the schedule for your event.
  • Waiver/Registration Form (pdf) -- This form is used by participants to register for most local events.

  • New Waiver Form for Minors - This article explains how you can participate in our events without a parent or guardian accompanying you.
  • Waiver and Permission to Participate (pdf) -- This is the waiver form for parents or legal guardians of minors under 18.  It is valid from the date of completion until the following end of July.
